Caribbean Metastatic Breast Cancer Care Navigation Program

“Focusing on the holistic needs of our MBC Caribbean people”

Care navigation is an integrative, multidisciplinary strategy that provides holistic patient care through intensive case management and education to empower patients to achieve enhanced quality of life.

In the Caribbean, there are very limited resources available to serve the needs of persons suffering from metastatic disease. The Caribbean Care Navigation Program (CNP) for metastatic breast cancer patients is designed to provide comprehensive care for individuals who are navigating life with MBC, focusing on improving quality of life, managing symptoms, and providing holistic support.

The program aims to help individuals cope with the physical, emotional, and psychological challenges posed by cancer, including those related to the progression of the disease, treatment, and long-term survivorship. The program will be customized from MBC patients’ lived experiences, combined with expert contributions from our regional research team.

Psychosocial Development

This module focuses on the psychosocial challenges that can arise from a metastatic breast cancer diagnosis. Topics include:

  • Relieving stress strategies
  • Navigating the healthcare system
  • Understanding the body’s response to illness and coping
  • Depression in cancer
  • Accessing supportive services
  • Introduction to the Pfizer LivingWith App

Physical Fitness & Exercise

This module offers evidence-based activities to ease cancer symptoms:

  • The role of physical activity in symptom relief
  • The impact of exercise on mental distress
  • Lymphatic yoga to reduce lymphedema

Spiritual Wellbeing

This module explores how spirituality and faith enhance resilience and coping strategies. One-on-one counselling is available throughout the program.

Nutrition

Focuses on nutrition to support patients with metastatic breast cancer:

  • Personal nutritional assessments
  • Balanced eating and well-being
  • Macro and micronutrients
  • Debunking diet myths
  • Meal tips and food shopping on a budget

Symptom Management

This module helps patients and caregivers manage symptoms related to MBC that impact quality of life. Led by experts in home care and hospice, it focuses on education, symptom relief, and emotional support.

Supportive Care Services

This session clarifies the differences between palliative and hospice care and how/when to access them. Includes time for Q&A tailored to your needs.

Caregiver Support

Caregivers are essential. This module equips them with tools, guidance, and emotional support to balance caregiving with self-care, helping create a compassionate support system for those living with MBC.

Legal & Financial Planning

Helping patients plan for the future with topics like:

  • Financial & asset management
  • Insurance options
  • Estate planning
  • Available legal services
